The PD requested the LI to walk the planned school-age playground. The use of fencing to establish boundaries and using one section of the parking lot was discussed.
An unannounced monitoring inspection was conducted in the Center on November 18, 2015. Upon arrival at approximately 11:15am, preschool children were in care. The children had eaten lunch and were on their cots for rest/nap time. The preschool program is conducted in 5 classrooms and the school-age program is on the upper level. The PD reported 109 children are currently enrolled in the program. The ages of the children currently attending the Center range from two years to 11 years-old. In the afternoon, children were also observed during outdoor play on the fenced playground. A sample of five records was reviewed. Documentation of the emergency practiced drills was reviewed. A discussion was held with the PD regarding the conducted drills. All children in care shall participate in the drills. A discussion was held with the PD regarding background check requirements for staff and Board officers. Findings of the inspection were reviewed with the PD upon exit. Standard #: 22VAC40-185-160-C Description: Based on documentation reviewed, the Center failed to ensure TB screenings for staff were current at least every two years from the date of the first initial screening. Evidence: The LI reviewed a sample of two records, documentation for staff #2 was dated 06/04/12. Reportedly the staff member is used as a substitute in the Center. Plan of Correction: The PD will inform staff to schedule an appointment to update the TB screening. Records will be monitored on-going to ensure documentation is kept current.
Standard #: 22VAC40-185-350-A Description: Based on the LI's observations, the Center failed to maintain required ratios while children were on outdoor playground.
The LI observed children from 5 classrooms on the fenced outdoor playground. There were 4 teachers with the children, their ages ranged from 2years to 4 years-old. The Teacher from one of the classrooms had stayed indoors to assist a child whom had gotten sick to their stomach. The LI observed one of the four teachers leave the playground with one child to take indoors for toileting. This left the group of 35 children with 3 teachers thus
exceeding the ratio requirements of 1 staff to 8 children.
Plan of Correction: Each Teacher will be instructed to take their group of children to the restroom at the same time unless other staff coverage for the group is in place at the time to maintain required ratios.
Standard #: 22VAC40-191-60-C-1 Description: Based on reviewed documentation, the Center failed to obtain a original criminal history record report within 30 days of the staff member's employment or volunteer service. Evidence: The LI reviewed a sample of two staff records, one of two did not contain documentation of a verified criminal history record report. Staff #1 was employed in the Center in July, 2015. Plan of Correction: The PD will notify the staff member that she can not work with the children until a criminal record check has been verified.
Standard #: 22VAC40-191-60-C-2 Description: Based on documentation reviewed, the Center failed to obtain verification of a Search of the Central Registry for staff #1 whom has been employed in the Center over 30 days, the date of employment was July, 2015. Plan of Correction: The PD will notify the staff member that she can not work with the children until a Search of the Central Registry has been verified.
