Comments:
An unannounced monitoring inspection was conducted on 10/15/15 from 10:42am - 12:15pm. During the inspection there were 21 children ages one to five years old in care with seven staff. A tour of the facility was conducted and children were observed in a variety of activities in the classrooms including arts and crafts, writing, playing with blocks, sensory activities and story time, and eating lunch. Records were reviewed for five children and two staff. There is no medication at the child day center. Emergency procedures, and emergency supplies were reviewed. Areas of non-compliance are identified on the violation notice, and were reviewed during the exit interview. lease complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to me within 10 calendar days from today. You will need to specify how the deficient practice will be or has been corrected. Just writing the word ?corrected? is not acceptable. Your plan of correction must contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventive measure(s).

Standard #: 22VAC40-185-130-B
Description: Based on a review of five children's records, it was determined that the facility did not ensure that there was documentation of additional immunizations is obtained once every six months for children under the age of two years. Evidence: 1. The record for child #2 (date of birth 12/2/12) contain documentation of a record of immunization that was dated 12/16/13. 2. Staff #1(Program Director) confirmed that an updated immunization record had not been

Plan of Correction: The parent of child #2 will be asked to provide an updated immunization record. All children's records will be reviewed to ensure they are up-to-date.

Standard #: 22VAC40-191-60-C-1
Description: Based on a review of two staff records, it was determined that the facility did not deny continued employment of a staff who did not have a criminal history record report within 30 days of employment. Evidence: 1. The record for staff #2 (date of hire 10/19/15) did not have documentation of a completed criminal history record report. 2. Staff #1 (Program Director) confirmed that she had not received the criminal history record report for staff #2.

Plan of Correction: The Program Director will call the state police to check the status of the criminal history report for staff #2. If it has not been received a new criminal history record report will be conducted.

Standard #: 22VAC40-191-60-C-2
Description: Based on a review of two staff records, it was determined that the facility did not deny continued employment of a staff who did not have a search of the central registry finding within 30 days of employment. Evidence: 1. The record for staff #2 (date of hire 10/19/15) did not have documentation of a completed search of the central registry finding. 2. Staff #1 (Program Director) confirmed that she had not received a completed search of the central registry finding for staff #2.

Plan of Correction: The Program Director will call to check the status of the search of the central registry finding for staff #2. If it has not been received a new search of the central registry finding will be conducted.
