Thank you for your time and assistance with the unannounced complaint inspection conducted on November 19, 2015 and December 1, 2015. The 12/01/2015 on-site visit was from approximately 2:30 p.m. to 4:20 p.m. The inspection was in response to a complaint that was received in the Valley Licensing Office on November 17, 2015. The complaint was in regards to supervision of children in care. The preponderance of the evidence gathered during the investigation did support the allegation and the complaint disposition was, therefore, "valid." The violation resulting from the visit can be found on the violation notice and the risk rating was noted on the supplemental page. At the time of the visit on 12/01/2015, there were twenty-six (26) children present and in the care of seven (7) staff members. The total enrollment for the program was sixty-six (66) children. There were six (6) staff members interviewed during the 12/01/2015 visit and two (2) children 's records were reviewed. Based on the repeat violation in this area of the standards, an Intensive Plan of Correction (IPOC) was issued. Description: Based on staff interviews, review of staff written accounts, and verification from staff members, it was determined the facility failed to ensure the care and protection of two children when they entered the building from the playground without the knowledge of their teachers. Evidence: 1. On 10/29/2015, at approximately 5:00 p.m., Staff Member #1 and Staff Member #2 were supervising a group of children on Playground A. Child A and Child B entered Classroom A through the door on the playground without the knowledge of the two staff members. Once inside, the two children were spotted by a third staff member and escorted back to the playground.
