Evidence: Child #3 has a start date of 9/8/15 and there is no documentation of a physical examination within 30 days of their start date. Plan of Correction: We will ask the parents to provide us with this documentation.

Standard #: 22VAC40-185-560-G Description: Based on direct observation and discussion, food brought from home is not labeled with children's names and the date. Evidence: Children's lunch boxes/bags do not all have a name on them indicating the owner. Food containers brought from home also do not have the child's name on them or the date. Plan of Correction: We will send out a letter to parents telling them that we will begin requiring this when school starts again in January.
