- Technical Assistance:
Conducted an unannounced monitoring inspection. Observed 35 children + 6 direct-care staff. Ratios and supervision were in compliance. Additional staff were on-site and available to assist in classrooms as needed. Since the last inspection a new Director + Assistant Director have been transferred to this location. Children were observed having story time, indoor/outdoor free play, and eating lunch during the inspection. Staff were actively engaged with the children, leading activities, and assisting them with tasks as needed. Infants were being held, fed, napping, and having play time on the floor. The facility was found to be cleaner than previously noted, safeguarded appropriately, and sufficiently supplied with toys and equipment for the children. Hazardous items and needed maintenance previously cited have been completed. Lunch served: hamburger soup with corn + green beans, tortillas, mango, and milk. No medications reported to be on-site. Standard #: 22VAC40-185-130-B Description: Based on records reviewed, it was determined that 1 of 2 children's records did not include documentation of immunizations every 6 months until the age of 2 years old. Evidence: Child #1 - Date of birth is 10/30/14, and the most recent immunizations on-file were dated as completed in March 2015. Plan of Correction: Parent was notified and latest shot record from 12/15 was given to management on 12/17/15
Standard #: 22VAC40-185-550-M Description: Based on records reviewed, it was determined that 1 of 10 written injury reports was incomplete. Evidence: One report did not include documentation of the time or how the parent was notified of the child's injury. Plan of Correction: Both managers will check accident reports before they are filed to make sure they are complete.
Standard #: 22VAC40-191-60-B Description: Based on records reviewed, it was determined that 1 of 3 staff records did not include a completed Sworn Disclosure Statement (SDS). Evidence: Staff #4 - Date of hire was in September 2015, and the staff person has been permitted to work in a direct-care role without a completed SDS. Plan of Correction: Form was given to employee to fill out on 12/15/15. It is in her file.
