Long-term prescription medications may be allowed with written authorization from both the child's physician and parent.
An unannounced monitoring inspection was conducted today from 12:25pm-1:30pm. There were 44 children (infants-5yrs.) directly supervised by 11 staff. The physical plant, 4 staff records, 4 children?s records, medications, and emergency supplies were inspected. Children were observed playing outside and preparing for lunch and nap. Areas of non-compliance are identified in the Violation Notice. Standard #: 22VAC40-185-150-B Description: Based on record review, a child's immunization report was not signed by a physician, his designee, or an official of a local health department. Evidence: The immunization report observed for Child 3 was not signed by the physician. Plan of Correction: The immunization report observed for Child 3 was signed by the physician. Parents are both physicians and the form was given t (sic) them to have signed.
Standard #: 22VAC40-185-60-A Description: Based on record review, two of four children's records reviewed were incomplete. Evidence: An annual update of children's information was not observed documented for Child 1 (last completed July 8, 2013) and Child 2 (last completed November 10, 2012). Plan of Correction: Both parents updated the forms and there were no changes, we update these forms throughout the year when parents change any information like telephone contact or address.
Standard #: 22VAC40-185-70-A Description: Based on record review, one of four staff records reviewed was incomplete. Evidence: Documentation of at least two references was not observed for Staff B, who was hired October 19, 2015. The reference form observed had been partially completed, but reference results were not documented. Plan of Correction: Staff B references call-back was on the call-back folder, verified the references and responses of good worker are on file.
Standard #: 22VAC40-185-280-B Description: Based on direct observation, hazardous substances were not kept in a locked place that prevents access by children. Evidence: A spray bottle labeled "chlorox & water" was observed in an unlocked cabinet in the two year old bathroom. Plan of Correction: Emphasized and retrained staff on our policy to keep all hazardous substances in the locked cabinet provided even if they stop cleaning for a minute or leave the cleaning supplies in a high cabinet out of reach of children. These should always be in the locked cabinet.
Standard #: 22VAC40-185-510-C Description: Based on medication review, the center did not follow their medication policies and procedures. Evidence: 1. A medication was observed on site for Child 5 with an expiration date of July 2015. 2. A medication was observed on site for Child 6 with an expiration date of June 2015. Plan of Correction: Medications discarded and/or returned to the parents appropriately (medications are never used if expired).
Standard #: 22VAC40-185-510-N Description: Based on medication review, when an authorization for medication expired, the center did not notify the parent that the medication needed to be picked up within 14 days or have the parent renew the authorization. Evidence: The written medication authorization form observed for Child 7 expired August 7, 2015 and a new authorization form was not obtained. Plan of Correction: Medications returned to parents until they bring an updated authorization form, teachers with a MAT certificate always double check the form before giving any medication and if not complete or expired, they call parents for a new one.
Standard #: 22VAC40-80-120-E-2 Description: Based on direct observation, the findings of the most recent inspection of the facility were not posted in the center. Evidence: The inspection findings observed posted in the center were from the December 18, 2014 inspection, which was not the most recent inspection of the facility. Plan of Correction: Copy was moved to show parents during tour and reposted on 12/7/15 and replaced with new findings on 12/9/15.
