Technical Assistance:
Discussed the following standards: background checks, staff records, children's records, medication, operational responsibilities, parental agreements, immunizations and physical examination for children, staff qualifications and training, physical plant, staffing and supervision, and special services.

Comments:
An unannounced monitoring inspection was conducted on 11/23/2015, and 12/15/2015. Observed 4 children directly supervised by 1 staff member on 11/23/2015. Ratio and supervision were in compliance. A sample of 2 staff records, and 2 children's records were reviewed. The physical plant, medications, emergency supplies and procedures were also reviewed. The children were observed during homework time. Areas of non-compliance are identified in this report. The exit interview was conducted with the Director. Description: Based upon documentation reviewed, children's records were incomplete. Evidence: 1) Two of the two children's records reviewed did not contain a written agreement between the parent and the center for an authorization for emergency medical care should an emergency occur when the parent cannot be located immediately (Child A and B). 2) Two of the two children's records reviewed did not contain documentation of child updates and confirmation of up-to-date information; last reviewed was done during school year 2013-2014 (Child A and B).

Description: Based upon documentation reviewed and direct observations, an employee was permitted to work in a position involving direct contact with children without a completed criminal history record (CRC) check on file. Evidence: 1) Staff B's record did not contain a CRC on file (Date of hire (DOH); 09/08/2015). 2) Staff B was directly supervising 4 children alone during the inspection on 11/23/2015. Staff B was not under the direct supervision of another employee for whom a background check has been completed.
