A complaint investigation was initiated on 11/19/15 regarding an allegation of insufficent staffing. The licensing inspector conducted an unannounced complaint inspection in response to a complaint that was received on 10/20/15. The information gathered from interviews with staff, observations and record review during an unannounced inspection, supports the allegation. This complaint is determined to be valid. The areas of noncompliance are identified on the violation notice. Please complete the columns for ?actions to be taken? and ?date to be corrected" for each violation cited on the violation notice. Include steps to correct the noncompliance with the standards; measures to prevent the noncompliance from occurring again; and person responsible for implementing each step and/or monitoring any preventive measures. Please return the dated, signed copy to the licensing office within 10 calendar days of receipt of this notice. If you have any questions, please call the licensing inspector.

Description: Based on observation, interview and a review of records, the facility did not ensure that when children are regularly in ongoing mixed age groups, the staff-to-children ratio applicable to the youngest child in the group shall apply to the entire group. Evidence: 1. On 10/8/15, there were 9 children in care in one room with 2 staff, instead of 3 staff, with children ages 7 months to 5 years in one group.

Plan of Correction: The center have hired new staff members to help with increased enrollment and have also adjusted routines where infants/toddlers are no longer combined with older age groups in order to prevent recurrences of ratio related problems within the facility

Description: Based on observation, the facility did not ensure that for children from birth to the age of 16 months, there was one staff member for every four children. Evidence: 1. On 11/19/15, at 7:23 a.m. there was one staff member with 5 infants in the infant care room when one parent left. The inspector reported this to the administrator.

Plan of Correction: We have adjusted teachers schedules to ensure that administration has the flexibility of being in the office for official business
