22VAC40-191 Background Checks (22VAC40-191)
An unannounced monitoring inspection was conducted today, January 7, 2016. Violations from the previous inspection have been corrected, however, a new violation notice was completed with one of the three violations being a repeat violation. Upon entering the center the infants have been moved to another room while the carpet is being removed from the infant room. Each class was either reading books, playing with the available toys, singing, or preparing for lunch. Lunch consisted of tuna & crackers, corn, orange slices and milk. Medications were also observed. Staff and children's records were reviewed for compliance. Should you have any questions please do not hesitate to call. Inspection began: 10:10 am and ended 1:30 pm, approximately.
Standard #: 22VAC40-185-270-A Description: Based on observation of the building the center failed to maintain some areas in a safe and operable condition. Evidence: 1. In the toddler 1 room the plastic molding located next to the large sink was missing and looked as though when it was pulled off the sheet rock peeled off with it. The preschool 1 room has plastic molding that is pulling away from the wall and there is sheet rock peeling next to the outside door. 2. In the school age room and the four year old room there are screens on the inside of the window that have holes in them, accessible to the children. 3. In the four year old room the rest mats are torn, exposing the inside padding. 4. On the playground in several areas the ground covering is exposed and it could cause a tripping hazard. Plan of Correction: During the inspection maintenance repaired the plastic molding in the toddler room. A work order has been placed with maintenance to repair the walls, remove the screens and work on the exposed ground covering. The rest mats will be replaced. The director, owner and staff will ensure they watch all areas of the center and request that repairs be made in a timely manner.
Standard #: 22VAC40-185-280-B Description: Based on observation during the inspection the center failed to keep all hazardous substances locked. Evidence: 1. The closet in the four year old classroom has a can of disinfectant spray on a shelf, not locked. Plan of Correction: The director will remind all staff that all hazardous substances are kept locked and not accessible to the children when not in use. Staff will ensure that all hazardous substances are kept locked when not in use.
Standard #: 22VAC40-185-500-A Description: Based on observation during the inspection the center failed to ensure that hands are washed after coming in contact with bodily fluids. Evidence: 1. While in the toddler room staff #2 wiped a child's nose and did not wash her hands afterwards. Plan of Correction: The director has talked with the staff and will ensure that all staff are reminded that hands are to be washed and when.
