Comments:
An unannounced monitoring inspection was conducted from 9:00 a.m.-11:30 a.m. with the program manager. 47 children, ages 4 months through 8 years old, were in the care of ten staff. Children were observed having tummy time, crawling, painting puffy snowmen, cutting and pasting the letter D dinosaurs, eating morning snack, playing Uno, and doing an "I Spy" book picture search. Morning snack served today was Cheese Nips and milk. The last fire drill was 1/21/16. The last shelter-in-place drill was 1/22/16. Two medications were reviewed and complete. Two staff records were reviewed. Three children's records were reviewed and complete. Violations:
Standard #: 22VAC40-185-160-A
Description: Based on review of two staff records, the center did not have documentation of staff's tuberculosis test or screening within 21 days of hire or within 12 months prior to hire. Evidence: Staff A's (hired 9/3/15) tuberculosis test was dated 11/20/15.

Plan of Correction: The program manager indicated that the center will be mindful of initial and repeat tb tests and screenings for current and future employees.

Standard #: 22VAC40-185-80-A
Description: Based on review of written classroom attendance records, the center did not maintain an accurate written record of children in attendance. Evidence: On 1/29/16, there were no children marked as present on the written attendance record in the infant classroom. However, there were eight infants physically present in the classroom.

Plan of Correction: The program manager spoke with the teacher who updated the attendance record during the inspection.

Standard #: 22VAC40-185-180-D
Description: Based on review of two staff records, the center did not have a driver complete a traffic violation disclosure form. Evidence: Staff A, who regularly transports students, did not have documentation of a form that discloses past moving traffic violations and intent to disclosure any future traffic violations.

Plan of Correction: The program manager typed a driver disclosure statement and had the teacher sign it during the inspection.

Standard #: 22VAC40-185-500-B
Description: Based on observation, the center did not have a diapering surface that was nonabsorbent. Evidence: The diapering pad in the infant room had four small tears on its surface.

Plan of Correction: The program manager will replace that diapering pad with an extra one from the center. She and the center director will review more durable diapering pad options to order in the immediate future.
