Comments:
Conducted an unannounced monitoring study with the assistant director, twelve staff, and 38 children. The ages of the children range from two months-old through five years-old. Three staff records and five children's records were reviewed as well. There were no children on medication today. The children were eating lunch and napping during the inspection. Start time: 10:10am End time: 12:20pm

Standard #: 22VAC40-185-160-C
Description: Based on a direct review of three staff records, one staff record was incomplete. Evidence: Staff A's tuberculosis screening was completed on 12/6/13 and was due on or before 12/6/2015.

Plan of Correction: The assistant director will have Staff A complete the tuberculosis screening this week.
