Reviewed over standard 22 VAC 40 -185 -60 and 22 VAC 40-185- 210 A 4 .

Comments:
An unannounced monitoring inspection was conducted on February 10, 2016 from approximately 10:00 AM to 12:15 PM. There was 7 staff were present with 24 children. Children were observed during clean up time, morning activites and name recognition. Five children records and one staff record on this date. Licensing Inspector reviewed supervision, activities, equipment, diapering procedures, hand washing procedures, nutrition, special feeding, injury report documentation, children's records, emergency evacuation and shelter-in-place drill documentation, health inspection report, emergency supplies and required positing. No medication is given at the center. The exit interview conducted with the Owner/Program Director.

Violations:
Standard #: 22VAC40-185-140-A
Description: Based on 1 of 5 children's records reviewed, the licensee did not ensure the records contained a physical examination. Evidence: During the inspection conducted on February 10, 2016, the records for child #2 did not have a physical examination.

Plan of Correction: The owner will have the mother contact the school for the physical.

Standard #: 22VAC40-185-70-A
Description: Based on 1 of 1 staff records reviewed and staff interview, the licensee did not ensure each staff record contained all required documentation. Evidence: During the inspection conducted on February 10, 2016, there was no written information to demonstrate that staff #1 possess the education, certification and/or experience required for the job position.

Plan of Correction: The owner will have documentation in the record in 30 days.

Standard #: 22VAC40-185-280-B
Description: Based on observation, the licensee did not ensure that all hazardous substances were kept in a locked place. Evidence: During the inspection conducted on February 10, 2016, in the following hazardous substances were observed in the classrooms: In the toddler classroom, on the desk there was bottle of hand sanitizer with " warning" and "keep out of reach of children." In the four year old classroom, on the sink there was a bottle of hand soap labeled with " danger " and "keep out of reach of children." In the three year old classroom bathroom, there was a bottle of hand soap labeled with "warning "and " keep out of reach of children."

Plan of Correction: The hazardous substances were removed during the inspection. The owner will be refilling empty bottles with soap that does not have warning labels.

Standard #: 22VAC40-185-500-B
Description: Based on observation, the licensee did not ensure the diapering surface is nonabsorbent surface. Evidence: During the inspection conducted on February 10, 2016, Licensing Inspector observed multiple tears on diapering pad in the Toddler's room which caused the surface to be absorbent.

Plan of Correction: The diapering pad will be removed immediately.
