Standard #: 22VAC40-185-400-A Description: Based on interviews conducted, it was determined that a staff person did not interact with a two-year old child in a manner that provided needed help, comfort, and support. Evidence: 1. On 2/3/16, at approximately 3:30pm, a crying child was not provided comfort by staff. After being disciplined inappropriately by Staff #1 the child was left to cry. 2. The witness and Staff #1 both reported that the child was crying, and that staff did not comfort him. Staff #1 stated "I told him to go play with centers." 3. The witness, who observed the incident from the window in the hallway, entered the classroom and comforted the crying the child. Plan of Correction: Staff have been retrained on how to properly provide help, comfort and support to children of all ages. Staff person was terminated. Classrooms will be monitored by management daily to ensure that staffs are providing the comfort and support to children throughout the day during times when children are in distress as well as when calm.
Standard #: 22VAC40-185-400-B Description: Based on interviews conducted, it was determined that a staff person did not provide behavior guidance that was constructive in nature, appropriate, or intended to redirect the child to appropriate behavior and resolve conflicts. Evidence: 1. On 2/3/16, at approximately 3:30pm, a child was observed by a witness walking toward the sink in the classroom with a paper towel in his hand when Staff #1 approached him and said "put it in the trash" in a "stern" and "loud" tone of voice. 2. Staff #1 reported that the child had been "messing with the paper towel dispenser" and ran away from her when she previously had instructed him to throw the paper towels in the trash can. Staff #1 stated "I said it in a stern voice." 3. Staff #1's voice was loud enough for the witness to hear from the hall, who was watching through the classroom window, despite the classroom door being closed. Plan of Correction: Staff members have been retrained on positive behavioral guidance. Staff person was terminated. Classrooms will be monitored by management daily to ensure staffs are following positive guidance guidelines. Training on positive guidance will occur once monthly during staff meetings.
Standard #: 22VAC40-185-410-1 Description: Based on interviews conducted, it was determined that a staff person roughly handled a two-year old child. Evidence: 1. On 2/3/16, at approximately 3:30pm, Staff #1 was observed by a witness "forcefully" grabbing a child by the upper arm as she told him to throw a paper towel in the trash can. 2. Staff #1 reported that she stepped in front of the child to block him, and "touched" his arm in an effort to stop him from running. 3. The witness and Staff #1 both reported that the child was crying. Plan of Correction: Staff members was terminated and all staff have been retrained on poistive guidance. Staff will attend positive guidance techniques trainings twice yearly.
