Comments:
Licensing Inspector arrived at 10:00 and departed at 11:45 AM. At the time of the tour, there were 69 preschool aged children in care with eight staff. Children were observed playing with toys, doing art activities and playing outside. Medication was also checked.

Violations:
Standard #: 22VAC40-185-60-A
Description: Based on a review of five children's records, it was determined that the facility did not ensure that all required elements were present in each child's record. Evidence: The record for child #2 did not contain the work phone number or place of employment for either parent.

Plan of Correction: Family Involvement Specialist will obtain information from parent and place in record.
