Comments:
The licensing inspector conducted an unannounced complaint inspection in response to a complaint that was received by the licensing department on 02/08/2016. The inspection was initiated on 02/12/2016 and completed on 02/19/2016. The licensing inspector was onsite from approximately 2:10pm until 3:30pm on 02/12/2016. On 02/19/2016, the inspector was on site from approximately 11:00am until 12:40pm. Interviews with staff were completed, observations were made in the center, and documentation was reviewed. The preponderance of the evidence gathered during the investigation supports the allegation; therefore, the complaint is determined to be valid. See the violation notice for more details. Complaint related: Yes
Description: Based on interviews and review of center policies, the licensee did not ensure that they comply with their own policies and procedures required by the Child Day Center Standards regarding bottle feeding Infants. Evidence: The facilities Infant feeding policies and procedures state: "Bottle Feeding and Infant Mealtime: Before feeding the child, match the label on the bottle to the child?s face. Confer with your co-teacher that you have the correct bottle and the correct child.?. Staff #1 stated on 02/03/16 two bottles for Infant A and Infant B that were the same make were being heated at the same time. Staff #1 stated she picked up the bottle belonging to Infant B and offered it to Infant A. Staff #1 stated after just a two or three seconds, Staff #2 said ?isn?t that Infant B?s bottle?. Both Staff #1 and Staff #2 stated the bottles for Infant A and Infant B were labeled correctly. Staff #1 said that she did not follow the procedures and policy of matching the label on the bottle to the child?s face.

Plan of Correction: Bottle feeding procedures were reviewed with all teachers. Before feeding the child, match the label on the bottle to the child's face/ Confer with the co-teacher that you have the correct bottle and the correct child.

Description: Based on interviews, the center did not to provide adequate care and protection for an infant in care that received another child's breast milk. Evidence: Based on interviews, the following was determined: 1. On 02/03/16 at approximately 9:15am Infant A drank a ?swig? of a breast milk from bottle belonging to Infant B. 2. Staff #1 stated two bottles that were the same make were being heated at the same time. Staff #1 stated she picked up the bottle belonging to Infant B and offered it to Infant A. Staff #1 stated after just a two or three seconds, Staff #2 said ?isn?t that Infant B?s bottle?. Staff #1 stated Infant A probably only took a swig of the milk. 3. Staff #2 stated on 02/03/16 two bottles that looked similar were being warmed at the same time. Staff #2 stated Staff #1 grabbed a bottle and began to feed Infant A for just a couple of seconds. Staff #2 stated she said ?isn?t that the wrong bottle?. 4. Both Staff #1 and Staff #2 stated a member of management was informed right away and parents of both Infants were contacted.

Plan of Correction: Bottle feeding procedures were reviewed with all teachers. Before feeding the child, match the label on the bottle to the child's face/ Confer with the co-teacher that you have the correct bottle and the correct child.

Complaint related: Yes
Description: Based on interviews and record review, the licensee did not ensure the required staff to child ratio for two year olds was being followed. Evidence: During the inspection, the licensing inspector reviewed the February 4, 2016 attendance record for Discovery Preschool classroom (Two Year Olds). The attendance record indicated that from approximately 2:30pm until 4:30pm there were 10 children present with one staff member. The required staff to child ratio was one staff member for every eight children present.

Plan of Correction: Schedules will be reviewed and adjusted as needed. Schedules are adjusted to the needs of the center and our families.
