Comments:
Today an announced initial study was conducted beginning at 10am. All areas could be inspected at this time. The center is changing ownership and still being operated under the current license until the new license takes effect. Children's records, 10 staff records (including new administration records) were reviewed. As a reminder, current daily health observation trainings begin expiring 3/12/16. The current director has scheduled this training for staff next week. There is one medication on-site with current required authorizations. Today there were 84 children present with 11 staff. Children were observed painting rainbows with gold coins at the bottom, having storytime, learning the letter "W" by making construction paper wagons with the letter, reading books, putting together puzzles and coloring. This inspection, including the exit interview, ended at 11:15am. Should you have any questions, please call.

Standard #: 22VAC40-185-60-A
Description: Based on record review, one child's record (#7) is missing complete required documentation in their record. Evidence: Child #7 is missing documentation of a physical examination in their record.

Plan of Correction: We will ask the parents to provide us with this documentation.

Standard #: 22VAC40-185-270-A
Description: Based on direct observation, there is a hazard in the 3's classroom. Evidence: Two sinks in the 3's classroom have chipping and peeling paint just behind the faucet area, accessible to children.

Plan of Correction: We will have this repaired by this weekend. We will make sure staff provide close supervision when children are washing hands.

Standard #: 22VAC40-185-280-G
Description: Based on direct observation, a substitute container did not have a label indicating its contents. Evidence: In the closet in the KG room, there was a spray bottle with a blue disinfecting solution in it. The spray bottle had no label.

Plan of Correction: We will make sure all substitute containers are labeled.
