Technical Assistance:
Discussion was held on the topics of current enrollment, current staffing, crib placement while infants are sleeping, and playground equipment.

The following inspection took place between the hours of 11:30 am through 2:30 pm. Today, I met with the Staff of the Kiddie Academy of Centreville located in Centreville, Virginia. At the start of this unannounced renewal inspection there were a total of 92 children with 15 staff; good ratios in all 6 rooms. A total of 3 children?s files and 3 new staff files were reviewed. The children were engaged in the following: toy play, diaper changing, hand washing, infant feeding, clean up, music, song, story time, lunch, nap time, and other organized activities. Standard #: 22VAC40-185-130-B
Description: Based on review, 1 of 3 children's files reviewed indicated the the center did not obtain documentation of additional immunizations once every six months for children under the age of two years. Evidence - 1. On the date of inspection (03/15/2016), the most current immunization page for Child #2 was dated - 06/29/2015. 2. Based on the information on file, on the date of inspection, Child #2 was under the age of two years.

Plan of Correction: An updated immunization page will be obtained and placed on file.

Standard #: 22VAC40-185-140-A
Description: Based on review, 1 of 3 children's files reviewed indicated that each child did not have a physical examination by or under the direction of a physician. Evidence - 1. On the date of inspection (03/15/2016), a Physical for Child #1 was not available for review. 2. Health forms on file at the center, had only a blank page where normal a physician would complete.

Plan of Correction: A physical for Child #1 will be obtained and placed on file.

Standard #: 22VAC40-185-60-A
Description: Based on review, 1 of 3 children files reviewed indicated that the center did not maintain all of the required information in the records of all enrolled children. Evidence - 1. On the date of inspection (03/15/2016), the Proof of Identification for Child #2 was not available for review. 2. The start date for Child #2 was listed as 08/26/2015.

Plan of Correction: Will obtain and place in file.

Standard #: 22VAC40-185-270-A
Description: Based on observation, a piece of equipment, outside of the center, was in an unsafe condition. Evidence - 1. On the date of inspection (03/15/2016), on the playground, the "sound machine," consisting of four PVC pipes and used as a drum had a broken piece along the lip of one of the tubes. 2. The broken lip now has an exposed jagged edge that is a possible cutting hazard.

Plan of Correction: This sound machine will be removed from the playground. If possible, the broken piece will be replaced and the children will be encouraged to play on the other sound machine on the same playground.
