Comments:
An unannounced inspection was conducted today in response to a self-reported incident regarding supervision of children. Based on interviews conducted and documentation reviewed, a preponderance of evidence supports a violation for supervision and is identified in the Violation Notice. Standard #: 22VAC40-185-340-F
Description: Based on staff interviews and documentation review, the center did not ensure that children under 10 yeas of age was always within actual sight and sound supervision of staff. Evidence: 1. On February 19, 2016 at approximately 5:30pm, Staff A was moving a group of four children from the Intermediates classroom to the Pre-K classroom next door. Once Staff A was in the Pre-K classroom, after about 4 seconds, Staff A realized that Child 1 (3yrs.) was not with the group and immediately went out of the classroom and saw Child 1 standing in the hallway in front of the Pre-K 2 classroom. 2. Staff B walked out of the staff room and noticed Child 1 standing outside of the door of a classroom. After a few seconds, Staff B noticed Staff A come out of the Pre-K classroom to find Child 1, who was not within sight and sound supervision of staff at all times.

Plan of Correction: All staff have been retrained on proper supervision and account procedures to ensure compliance. Administration has observed all staff and will continue to do so in order to ensure that proper supervision and reporting procedures are being followed.
