Description: Based on observations made, it was determined that areas and equipment of the center, inside and outside, were not maintained in a safe and operable condition. Evidence: 1. Playground Climbing Structure - Rusted bolts, support posts/beams, railing, steps, and platforms were present on approximately 95% of the large climbing structure. The rusted bolt caps had been painted with silver metallic paint, but evidence of the rusted bolts were visible and no actual repair/replacement had occurred since this was cited in the 20/20/15 inspection report. The rubber/plastic coating on steps and platforms has broken on much of the surfaces, some with sharp edges, and exposing rusted metal underneath. The spiral slide panels are separating and have snag and pinch points. [Photos #2, #3, #4, #5, #6, #7, #8, #9, and #10] 2. Rugs/Floor - There was ground in black hard dirt/grime on carpeting in multiple classrooms along the edges where the carpet meets the tile. Additionally, the floor in the 2's classroom was dirty with paper trash, leaves/grass, a garment price tag, multiple hair ties, and a sock. 3. Broken Furniture - The base of a side panel on a bookshelf was broken in the 2's classroom. A piece of the pressed wood was missing. [Photo #1] A bookcase in the 4's classroom had a similar, but lesser broken bookcase base panel. It was also noted that approximately 7 other pieces of classroom furniture were currently being held together with tape. 4. Chipping Paint - Chipped paint was observed on the window ledges and mid-wall trim in the 2's classroom.

Plan of Correction: 1. A work order was placed on 4/18/16 asking for new bolts to replace rusted bolts on play structure. The facilities team was also made aware of repairs needed on the plastic coating on steps and platforms. The children will not use the spiral slide until fixed. Work order was placed to have slide panels checks + repaired (from seperating) sic 5/6/16 We corrected te violation of items on the two's floor right away by picking up/throwing away the loose items. 4/6/16 A work order was placed to get the wax from the floor tiles off the carpet. 5/6/16 The center has begun replacing shelves as needed once they are broken, or show signs of repair. 6/1/16 We have covered the window ledges + mid wall trim with a rubber coating to protect children from the chipped paint. 4/18/16
