Comments:
An unannounced monitoring inspection was conducted today, thirty four children and 3 staff were outside playing ball games, running and jumping rope at the beginning of the inspection. They returned to the cafeteria, washed hands and had snack ( cinnamon crisps, yogurt, carrots/dip and fresh fruit). Two children had medications on site. Three staff and four children's files were reviewed today and determined to be in compliance. Two classrooms were measure which will be used for the Summer Program. Classroom 27 has a capacity of 28 children and classroom 28 has a capacity of 30 children. Arrival time: 3:45 p.m. Departure time: 4:45 p.m.

Violations:
Standard #: 22VAC40-185-260-A
Description: Based on a review of the most recent fire inspection, it was determined that there was no annual fire inspection on file. Evidence: The last fire inspection is dated 9/15/14.

Plan of Correction: We will remind the school that we are in in need of the most recent fire inspection.

Standard #: 22VAC40-185-550-M
Description: Based on an interview, it was determined that the center is not documenting all the components of injuries/accidents. Evidence: The center is not documenting the date and time parents were notified of injuries, any future action to prevent recurrence of the injury, staff and parent signature or 2 staff signatures or how parents were notified of said injuries.

Plan of Correction: We will use the injury/accident form to document injuries/accidents to the children.
