Discussion about sight and sound supervision. Consult on rest time and ratios.

An unannounced monitoring inspection was conducted beginning at 11:00 a.m. and ending at 2:45 p.m. with the director and assistant director. There were sixty-five children in care with eight staff. Infants were observed having playtime on the floor and preparing for lunch. Diapering procedures were observed. Bottles were labeled and dated. Lunch today was Beefy Mac Casserole, green beans, pears, whole wheat bread and milk. The two's class was observed preparing for nap and resting. Diapering procedures were observed. The Toddler class was observed napping. The Prep (three's) class was observed preparing for rest and restroom time. Junior Kindergarten was observed preparing for rest and reading a group story. The other Junior Kindergarten class was observed during rest. No school age children were present during the inspection, however the classroom was observed. Standard #: 22VAC40-185-270-A
Description: Based on observation, not all areas of the center were maintained in clean, safe, and operable condition. Evidence: In both the infant room and older infant room, the blue fabric on the rockers is soiled and is visibly dirty. In the older infant room, the rocker has two tears in the seat cover. In the two's class, there are three areas of yellow peeling paint on the right wall at adult eye level. The fridge handle is broken and creates a scratching or pinch hazard.

Plan of Correction: The rockers were removed from the infant room during the inspection. New rockers will be put in the infant room. A new fridge will be obtained for the two's room.

Description: Based on observation, the center failed to follow required staff to children ratio requirements during rest period. Evidence: In the Prep class, eighteen three year old children were observed with one staff. Children were observed awake on their cots, at least six children were still taking bathroom breaks and at least two children had not settled on their cots. The second teacher returned within two minutes. Ratios should not be doubled during designated rest periods until the vast majority of children are settled on their cots and resting.

Plan of Correction: The teacher was spoken to immediately and reminded to contact management when they need a restroom break and to not double the ratio until all children are settled in the room.

Description: Based on observation, not all sunscreens were labeled with the child's name as required. Evidence: In the Toddler class, there was a "Waterbabies" brand sunscreen and a "Wegman's" brand sunscreen that were not labeled with the child's name.

Plan of Correction: The sunscreens were labeled during the inspection.

Description: Based on observation, not all diaper ointments were labeled with the child's name. Evidence: In the two's class, there was a "Desitin" brand diaper brand that was not labeled.

Plan of Correction: The ointment was labeled during inspection.

Description: Based on observation, the most recent violation notice from the 10/7/2015 inspection was not posted as required.

Plan of Correction: The violation notice was posted, we believe a parent may have accidently taken it.
