An unannounced monitoring inspection was conducted today April 26, 2016 to determine the facilities compliance with licensing standards. The inspector arrived at 10:30 am and concluded the inspection at 1:00 pm. Upon the inspector's arrival, children were observed in a variety of activities. Children had been playing in centers (house keeping, manipulatives, and blocks) and were preparing for lunch. Hand washing was conducted before heading to the cafeteria for lunch. The inspector observed lunch where staff sat at each table and assisted children with request to help open items. Staff also conversed with children and encouraged them to try items on their plate. After lunch children had time on the playground where they played on climbing equipment, basketball, with frisbees, sidewalk chalk and waffle ball. Staff covered all areas of the playground and participated in activities including tossing the ball for children to take a turn batting it. Upon returning inside children were eager to show the inspector a tree frog and a bug they had caught and placed in a habitat. Later children had nap time. Soft music was played in the background. Children who were not interested in napping were given books to read during the rest period. The classroom had age and stage appropriate materials. Children's art work and class work was displayed. Required information was posted to include the license, last inspection notice, daily schedule, emergency numbers, evacuation procedures/maps and parent information. The first aid kit and non-medical emergency supplies were inspected and found complete. The inspector interacted with staff and children. Three staff records were reviewed along with three children's records. One staff has current MAT. Staff have current daily health observation training along with current CPR and first aid training. Per the staff currently there is no medication being administered. The most recent evacuation drill was conducted on 03/01/2016. The two required shelter-in-place drills for the 2015-2016 school year were documented on 10/15/2015 and 01/29/2016. Today's lunch consisted of spaghetti, bread, green beans, mixed fruit and milk. See violation notice) Today's there were 18 children with two staff and one family advocate. Violations from the previous inspection on December 10, 2015 were checked for corrections. Standard #: 22VAC40-185-560-F-4 Description: Based on observation, the facility did not ensure a menu listing foods to be served for meals and snacks during the current week period was posted in a location conspicuous to parents. Evidence: There was no menu posted in the facility for parents. Plan of Correction: Per the director; I will obtain a month menu from the school and post it in the classroom on the parent board.
Standard #: 22VAC40-191-40-D-1-C Description: Based on review of staff records, the facility did not obtain a criminal history record check before three years since the date of the most recent criminal history record check report. Evidence: The criminal history record check for staff #3 was dated 02/16/2016, exceeding three years since the previous criminal history record check that was dated 01/16/2013. Plan of Correction: Per the director: We will review expiration dates and ensure background check request are completed in a timely manner.
