Comments:
An unannounced full renewal inspection was conducted today. There were 51 children in care, with a total of 5 staff members assigned to their direct supervision. The children were observed upon arrival to the after school program. After checking in, the children were observed decorating mugs, playing on the playground, listening to an outdoor safety reminder lesson, completing homework, and eating snack. The children were served apple slices, graham crackers, and milk for snack today. This inspection was completed between approximately 3:30 pm and 4:45 pm. Thank you for meeting with me today. Violations:
Standard #: 22VAC40-185-60-A
Description: Based on a review of children's records, it was determined that three records were not complete. Evidence: 1. Child record #1 does not include: each parent's work contact information; the address for the second emergency contact person; "authorized pick ups" information. 2. Child record #4 does not include a physical exam report. 3. Child record #7 does not include: "authorized pick ups" information; a physical exam report.

Plan of Correction: All parents will be asked for the missing information.

Standard #: 22VAC40-185-510-C
Description: Based on a review of children's medications, it was determined that one medication has expired. Evidence: One child's long-term prescription emergency medication expired on 4/30/16.

Plan of Correction: The parent will be asked to bring a new medication.
