Technical Assistance:
1. Discussed staffing plans to allow for timely diaper changing in the bathrooms, which were determined to be in compliance. A review with staff is to occur. 2. Reviewed appropriate cleaning supply storage.

Comments:
Conducted an unannounced monitoring inspection 11:00am-12:05pm. Observed 23 children + 6 direct-care staff + 1 administrative staff person. Ratios were in compliance and supervision was constant. Children were divided into age appropriate groups and were observed learning the Hebrew alphabet, coloring, and reading books. Staff were actively engaged with the children and leading the activities in each group. The center was found to be clean, safeguarded appropriately, and sufficiently supplied with toys and equipment for the children. According to the posted menus, cantaloupe was the morning snack. The outdoor playground was not inspected due to inclement weather. Medication management was in compliance. Staff training requirements for First Aid, CPR, MAT and daily health observations were met. Standard #: 22VAC40-185-160-C
Description: Based on records reviewed, it was determined that 2 of 4 staff records did not include a TB screening every 2 years. Evidence: The previous TB screenings for Staff #6 and Staff #7 were completed in December 2013.

Plan of Correction: One staff has already given in new TB screening 2nd staff will give in by the end of week [sic]

Standard #: 22VAC40-185-60-A
Description: Based on records reviewed, it was determined that 1 of 3 children's records did not include the name, address, and telephone number of 2 emergency contacts. Evidence: Child #14 - The addresses were not documented for the 2 contact persons listed.

Plan of Correction: Ask parent to update emergency contacts to include address info.

Standard #: 22VAC40-185-70-A
Description: Based on records reviewed and interviews conducted, it was determined that the education of a contract staff person permitted to be alone with children has not been verified. Evidence: Contract Staff #1 - The educational documents were not on-file, and the Director stated that she had not requested them.

Plan of Correction: Request hs proof from Joy of Dance instructor or have preschool staff stay with instructor [sic]
