Technical Assistance:
1. Discussed appropriate storage for personal bags belonging to staff. 2. Surfaces that are becoming rusted, or have chipping paint should be monitored and remedied accordingly. 3. Reviewed square footage, subsequent maximum capacities and age limitations for each building.

Comments:
Conducted an unannounced monitoring inspection 10:00am-12:50pm. Observed 61 children + 8 direct-care staff + 3 administrative staff. Ratios and supervision were in compliance in all groups. Staff were actively supervising the children, leading activities, and providing assistance as needed with tasks. Children were observed learning Spanish, having outdoor free-play, washing hands, and eating lunch. Lunch served: french toast, hash brown potatoes, bacon, syrup, banana, and milk/water. The center was found to be clean and well supplied with developmentally appropriate equipment for the children. The school bus was inspected today. Staff training for First Aid, CPR, MAT, and daily health observations were in compliance. Record keeping was well organized. Required documents were posted. Standard #: 22VAC40-185-270-A
Description: Based on observations made, it was determined that an area of the center was not maintained in a safe condition. Evidence: An exterior downspout located on the playground was loose and posed as a potential hazard. The metal bracket holding it onto the side of the building was broken, and it had become dislodged from the upper gutter connection. The inspector was able to move the downspout with ease when pushed. Children were observed on the playground during the inspection.

Plan of Correction: THE CHURCH WAS NOTIFIED OF THE HAZARD AND REPAIRED THE METAL BRACKET AND PIPE

Standard #: 22VAC40-185-280-B
Description: Based on observations made, it was determined that hazardous substances were not kept in a locked place. Evidence: A powder cleaning product and spray adhesive were found in an unlocked cabinet in Room #4. Children were observed in this classroom during the inspection.

Plan of Correction: THE HAZARDOUS SUBSTANCES WERE PLACED IN A LOCKED CABINET. STAFF WERE REMINDED THEY NEED TO BE LOCKED UP.

Standard #: 22VAC40-185-550-L
Description: Based on records reviewed, it was determined that a parent was not notified of a child's minor injury by the end of the day of occurrence. Evidence: 1 of 10 written injury reports indicated that the parents were notified the day after the injury had occurred.

Plan of Correction: STAFF WAS REMINDED ABOUT HOW TO FILL OUT THE ACCIDENT REPORT FORM APPROPRIATELY
