Comments:
The inspector conducted an unannounced monitoring inspection from 10:00 am until noon at the center and then continued the inspection by reviewing staff records at the main office from 12:30-1:15. There were eighteen children in care with staff maintaining ratios at all times. Children were observed playing in the gym, classroom activities, washing hands preparing for lunch, and at lunch. Two children's records and two staff records were reviewed . There was one repeat violation and others had been corrected. Please call your inspector with any questions at 276-698-9981. Thank you.

Standard #: 22VAC40-185-60-A-7
Description: Based on record review, the center did not obtain all the the required information for a children's file. Evidence: 1. The inspector reviewed two children's files. One of two files (child record #1) did not have the information listed for allergies completed.

Plan of Correction: The teacher will ask the parents for this information and update the form.

Standard #: 22VAC40-185-330-B
Description: Based on measurement, the center did not maintain the required depth of mulch. Evidence: 1. The inspector measured the mulch and under the swinging bridge the mulch measured between 2-3 inches.

Plan of Correction: The teacher will tell the maintenance worker. There is enough mulch to be redistributed to where it it is needed.
