22VAC40-191 Background Checks (22VAC40-191)
An announced record review was conducted on May 18, 2016. A sample of three staff records were reviewed. Areas of non-compliance are identified on the violation notice. The results of the inspection and violations cited were reviewed and verified with the Recreation Operations Coordinator during the exit interview. Please complete the columns for description of action to be taken and date to be corrected for each violation cited on the violation notice, and then return it to the licensing office within 10 calendar days of receipt. Standard #: 22VAC40-185-160-A Description: Based on a record review in one of three records reviewed, the licensee did not ensure that each staff submitted documentation of a negative tuberculosis (TB) screening. Documentation of the screening shall be submitted no later than 21 days after employment or volunteering and shall have been completed within 12 months prior to or 21 days after employment. Evidence: 1) During the inspection conducted on May 18, 2016 there was no documentation of a TB screening for staff #2 (employment date 1-15-16) submitted no later than 21 days after employment. Plan of Correction: The staff member has been removed from the site location until the TB test has been completed.
Standard #: 22VAC40-185-70-A Description: Based on a record review in three of three records reviewed, the licensee did not ensure each staff record contained all required documentation. Evidence: 1) During the inspection on May 18, 2016 the record for staff #1 did not include documentation of a complete address of a person to contact in the event of an emergency and information to be kept at the center about any health related problems that may interfere with fulfilling the job responsibilities. 2) The record for staff #2 did not include the documentation of the name, address and telephone number of a person to be notified in the event of an emergency, information to be kept at the center about any health related problems that may interfere with fulfilling the job responsibilities. 3) The record for staff #1 did not include documentation of a complete address of a person to contact in the event of an emergency. Plan of Correction: The emergency contact forms will be completed and files will be updated.
