Comments:
An unannounced monitoring inspection was conducted from 10:00 am - 2:15 pm. There were 74 children in care being supervised by 11 staff members. The children were observed participating in outdoor play, singing songs, and preparing for lunch. Lunch served today is macaroni and cheese, broccoli, strawberries, and milk. The most recent emergency escape drill was conducted 05/21/2016 and the most recent shelter in place drill was conducted 05/31/2016. Ten children's records were reviewed and in compliance. Five staff records were reviewed and in compliance. Eight medications were reviewed. There are five staff members who are MAT (Medication Administration Training) certified. Standard #: 22VAC40-185-270-A
Description: Based on observation of the center it was determined that not all areas of the playground and center were in clean and safe condition. Evidence: 1. The toilets located in the Pre-K classroom had urine on the seats additionally, the bathroom had a urine odor. 2. The LI (Licensing Inspector) observed a piece of metal protruding from the two year old fence that was accessible to children.

Plan of Correction: 1. The Pre-K bathrooms were cleaned during the inspection. 2. Maintenance repaired the fence protrusion during the inspection.

Standard #: 22VAC40-185-440-B
Description: Based on observation of the infant classroom and teacher interview it was determine that not all cribs were identified for use by a specific child. Evidence: Child A did not have an assigned crib labeled to identify the owner.

Plan of Correction: The label for child A with picture, first and last name will be posted on the designated crib.

Standard #: 22VAC40-185-510-C
Description: Based on observation of eight medications it was determined that not all medication authorization forms were updated every ten days. Evidence: The medication authorization form for child B for one medication was dated 05/09/2016.

Plan of Correction: Administration will request a new authorization form for the parent of Child B.
