Comments:
Thank you for your time and assistance with the unannounced monitoring inspection conducted on June 1, 2016. The on-site inspection was from 9:30 a.m. to 3:25 p.m. On this date, at the time of arrival, there were seventy (70) children in the care of thirteen (13) staff members. There were one hundred and five (105) children enrolled in the program. There were ten staff records and six children?s records reviewed. The medication and medication paperwork were reviewed for seven children. There were eight staff members and three children interviewed, and six classrooms and two playgrounds were observed. The exception to compliance was noted on the violation notice. Lunch consisted of chicken nuggets, ranch pasta salad, broccoli, peaches, rolls, and milk. All interactions between the staff and the children were age appropriate and positive. Outdoor play, choice time, infant feeding, diaper changing procedures, story time, hand washing and bathroom procedures, lunch, and nap time were some activities observed during the visit. Standard #: 22VAC40-185-130-B
Description: Based on review of six children's records and verification from the staff, the record for one child under the age of two did not include documentation of the additional immunizations the child had received after enrollment. Evidence: 1. The record of Child #3 (age 1 year 7 months) did not contain documentation that the child had received the required updated immunizations every six months until the child is two years of age. 2. Date of enrollment for Child #3 was 01/29/2015 and the one immunization record in the file was dated 12/19/2014.

Plan of Correction: The parent of the child was contacted during the inspection and the center will be sure to obtain the updated immunization record for the child as soon as possible. In the future, the center will ensure that all parents provide updated immunization records every six months for children under the age of two.
