Comments:
An unannounced monitoring inspection was conducted today at 11:30am. The physical plant, 2 staff records, 5 children?s records, medications (no new or expired medications from previous inspection), evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed participating in practical life, art activities, group activities, and eating snack. Hand-washing procedures were also observed. There was an abundant supply of books, toys, and materials for the children. The center was clean and organized. Areas of non-compliance are identified in this report. Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to me within 10 calendar days from today. Standard #: 22VAC40-185-280-B
Description: Based on observation, the facility failed to ensure that hazardous substances are kept in a locked place. Evidence: 1) Observed in the upper hallway, an unlocked cabinet with Comet, Clear Image spray, paint, and Spritz Sanitizer 2) Observed in Room 10 Oxivir, RTV Sanitizer and goo gone

Plan of Correction: Locked all of the cabinets in the hallway and Room 10. Staff have been reminded to keep closets locked at all times.

Standard #: 22VAC40-185-290-3
Description: Based on observation, the facility failed to ensure that electrical outlets have protective covers. Evidence: 1) Observed 1 uncovered electrical outlet in Room 1 2) Observed 2 uncovered electrical outlets in the Art Room. 3) Observed 2 uncovered electrical outlets in Room 11

Plan of Correction: Covered the otulets in ROom 1, 11 and the Art Room

Standard #: 22VAC40-185-430-B
Description: Based on observation, the facility failed to ensure that the materials and equipment available shall be age and stage appropriate. Evidence: 1) Observed that there were small pompoms on the shelf as an activity in Room 9 with children under the age of 3, causing a potential choking hazard. 2) Observed that there were small foam pieces on the fall that had fallen from art on the wall, causing a potential choking hazard.

Plan of Correction: These items have been removed from the classroom. The foam pieces have been thrown out.

Description: Based on observation, the facility failed to ensure that the diapering surface was nonabsorbent. Evidence: Observed that the diapering mat in Room 10 had a tear, making it an absorbent surface.

Plan of Correction: Diaper mat was replaced.

Standard #: 22VAC40-185-540-C
Description: Based on observation, the facility failed to ensure that the first aid kit included all required items. Evidence: Observed that the first aid kit was missing tweezers and triangular bandages.

Plan of Correction: We have updated the first aid kit with tweezers and triangular bandages.
