Violations:
Standard #: 22VAC40-185-340-A
Description: Based on a review of documentation and staff interviews, it was determined that on Thursday, May 12, 2016 staff members did not fully ensure one chid's care, protection, and guidance. Evidence: During the inspection, it was confirmed that on May 12. 2016 at approximately 9:05 a.m. staff member #1 fed child #1 a bottle of breast milk that was intended for use only by child #2. Staff #1 said "the bottle was in his (child #1) mouth for only 2 to 3 seconds and and then I immediately realized it". The bottle was intended for child #2 and not child #1. Center staff confirmed the bottle was dumped out and that "the bottle was practically full" said staff #2.

Plan of Correction: 22VAC40-185-(5)-340-A All caregivers have been re-trained on proper feeding procedures effective 5/26/16. All caregivers will verbally confirm a child's identity and the correctly labeled bottle with another tenured/veteran teacher effective 5/26/16. Management will observe daily to ensure compliance. .
