Comments:
An unannounced monitoring inspection was conducted on June 2, 2016. At the time of entrance there were 13 children and 3 staff members present. The sample of records reviewed consisted of 3 children's records and the emergency contact information and health related information for 3 staff. Children were observed playing pool and air hockey. The inspection began at approximately 3:50 p.m. and ended at approximately 5:15 p.m.. Areas of non-compliance are identified on the violation notice. The results of the inspection were reviewed with the center director during the exit interview. Additional program space (multipurpose room A, B and C ) were measured and inspected on this date at the request of the licensee. Please complete the columns for description of action to be taken and date to be corrected for each violation cited on the violation notice, and then return a signed copy to the licensing office within 10 calendar days of receipt. Violations:
Standard #: 22VAC40-185-60-A
Description: Based on a record review, in one of three records reviewed, the licensee did not ensure a separate record for each child enrolled was maintained at the center and contained all the required information. Evidence: During the inspection on June 2, 2016, the record for child #3 did not include the appropriate legal paperwork on file when the custodial parent requests the center not to release the child to the other parent.

Plan of Correction: Parent brought in the appropriate legal paperwork.

Standard #: 22VAC40-185-70-A
Description: Based on a review of required staff documentation to be kept at the center, the licensee did not ensure for each of the staff there was documentation of a person to contact in the event of an emergency and information about any health related problems that may interfere with the person fulfilling the job responsibilities. Evidence: During the inspection on June 2, 2016, three staff records were reviewed and one record, staff #3 did not include documentation regarding a person to contact in the event of an emergency, to include an address and phone number and information about any health related problems that may interfere with the person fulfilling the job responsibilities. .

Plan of Correction: Staff completed paperwork that same day.

Standard #: 22VAC40-185-280-B
Description: Based on observation, the licensee did not ensure hazardous substances such as cleaning materials, insecticides, and pesticides were kept in a locked place using a safe locking method that prevents access by children. Evidence: During the inspection on June 2, 2016, there was a spray container of disinfectant cleaner sitting on a shelf in the game room.

Plan of Correction: Center staff locked disinfectant away in another room.
