Comments:
An unannounced monitoring inspection was conducted today from 10:00am-12:30pm. There were 68 (18 months to 6 years) directly supervised by 8 staff. The physical plant, 2 staff records, 5 children?s records, medications, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed participating in individual and group play, and in music sessions with an independent contractor. There was an abundant supply of books, toys, and materials for the children. The center was clean and organized. Areas of non-compliance are identified in this report. Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to me within 10 calendar days from today. Standard #: 22VAC40-185-140-A
Description: Based on review, the facility failed to ensure that each child has documentation of a physical examination by or under the direction of a physician within 30 days after the first day of attendance. Evidence: Reviewed 5 children's records and found that CH1 did not have documentation of a physical examination.

Standard #: 22VAC40-185-230-A
Description: Based on observation, the facility failed to ensure that individuals from independent contractors shall not be counted in the staff-to-child ratios. Evidence: Observed that there was a contractor was used to maintain ratios during music sessions. There were 16 Primary children with 1 staff member and the music contractor.

Description: Based on observation, the facility failed to ensure that hazardous substances are kept inaccessible to children at all times. Evidence: Observed a bottle of bug spray accessible to children, in a child's cubby, marked Keep Out of Reach of Children.
