Comments:
An unannounced renewal inspection was conducted today, 7/5/16. Some of the violations from the previous inspection have been corrected. A violation notice was completed regarding the repeat violations and new violations found during today's inspection. Upon arrival to the center the school age class was outside on the playground and the younger classes began eating lunch. Following lunch the children had nap time. There are 38 children in care with nine staff available. Staff and children's records were reviewed for compliance. Should you have any questions please do not hesitate to call. Inspection began: 11:00 am and ended 2:15 pm approximately.

Violations:
Standard #: 22VAC40-185-60-A
Description: Based on review of children's records the facility did not have the required information in the file. Evidence: 1. 1 of the 5 records reviewed did not have the date the child entered care. (#2)

Plan of Correction: The director and owner will ensure that all information is included in the children's files. Corrected during inspection.

Standard #: 22VAC40-185-60-A-7
Description: Based on review of the children's records the facility did not include actions to take in regards to allergies listed. Evidence: 1. 1 of 5 records reviewed listed that a child had an allergy but there was no action to take listed should the child come in contact with the medication. (#4)

Plan of Correction: The director and administrator will review files and ensure that all information is included in the files. Corrected during inspection.

Standard #: 22VAC40-185-270-A
Description: Based on observation of the center during the renewal inspection the center did not maintain the areas in a clean and safe condition. Evidence: 1. The bathroom between the preschool rooms has a strong odor of urine. 2. The bathroom located in the hallway (WP287) had a large amount of toilet paper in the bowl that had not been flushed and the urinal had not been flushed. 3. Located on the toddler/two's playground is a down spout that is metal and has sharp edges. This is accessible to the children. 4. In the four year old room the rest mats are torn exposing the inside padding. (repeat violation)

Plan of Correction: The director and administrator will have staff clean the bathroom areas daily or more if needed. The bathrooms will be monitored to ensure that they are flushed. The downspout will have a piece added to it that will not be sharp. All of the areas will be monitored by staff and director to ensure they are kept clean and safe.

Standard #: 22VAC40-185-540-B
Description: Based on observation of the center and outdoor areas the facility did not keep the first aid kit inaccessible to the children. Evidence: 1. Located in the school age room was a first aid kit on a desk, accessible to the children. 2. Located on a table outside was a first aid kit accessible to the children.

Plan of Correction: Both first aid kits were removed during the inspection and placed in areas easily accessible to the staff but no the children. The director will monitor these areas to ensure safety.
