Technical Assistance:
Technical assistance was given regarding the clear posting of the results of the most recent inspection.

Comments:
An unannounced monitoring inspection was conducted on July 13, 2016. There were 4 staff were present with 44 children. Children were observed playing large group games in the gym nand working on an art project in the cafeteria. Children were also observed preparing for a swimming field trip. The sample size of records reviewed included 3 children's records and the health related information and emergency contact information for 4 staff members. One medication was reviewed as a part of the inspection. The violations from the previous inspection were reviewed and corrected. The inspection began at approximately 10:00 a.m. and ended at 11:40 a.m.. The results of the inspection were reviewed with the center director during the exit interview. The media room was also observed at the request of the licensee as it is now additional program space to be used as needed.

Violations:
Standard #: 22VAC40-185-60-A
Description: Based on observation and staff interview, the licensee did not ensure the center kept a separate record for each child enrolled which contained all the required information. Evidence: During the inspection on July 13, 2016, the records for the following children were determined to be incomplete; 1) The record for child #1, #2 and #3 did not include the start date the child began in the program. 2) The record for child #3 did not include the complete address for 1 emergency contact to include the street name, house number or zip code and did not include the home number or complete home address to incldue street name, house number or zip code.

Plan of Correction: The program director will have the records updated.
