Today an unannounced self-reported monitoring visit was conducted with the New Principal. This visit took place in the morning between 9:00am-12:30. There were 7 classrooms observed with a total of 110 children with 20 staff within the supervision guidelines. The children were observed having morning free play, arts & craft, water play outside, returning from a field trip, lunch and in transition for nap time. Positive interactions were observed during this inspection. A complete inspection of the physical plant, children and staff records, fire drill log, menu and emergency procedures were observed during this inspection. Standard #: 22VAC40-185-270-A Description: Based on observation of the physical plant, the following areas of the center were found not being maintained in a clean, safe and operable condition. Evidence: In the intermediate classroom, 6 holes were found in the bathroom along the wall. In the intermediate classroom, a divider was found in the bathroom with rust and chipped paint. in the Pk1 classroom, plastic borders were found outside of the boys and girls bathroom coming off of the hinges. Plan of Correction: Principal stated that a work order was placed last week and will follow up with maintenance.
Standard #: 22VAC40-185-280-B Description: Based on observation of the physical plant, a a hazardous substance was found on a cabinet accessible to children. Evidence: A deodorizer was found in the main lobby on a cabinet unlocked and accessible to children. Plan of Correction: Principal immediately removed the spray during this visit and placed it in a locked cabinet in the kitchen. Principal will do a walk through to ensure this items are not accessible.
Standard #: 22VAC40-185-280-G Description: Based on observation of the classroom, a substitute container did not include a label indicating its content. Evidence: in the toddler classroom, a spray bottle was found with a liquid substance however it was not clearly labeled to identify its content. Plan of Correction: Principal will ensure that all containers are labeled and placed in a cabinet with a lock on it.
Standard #: 22VAC40-185-280-H Description: Based on observation of the classroom, a purse was found accessible to children. Evidence: In the toddler classroom, a purse was found accessible to children . Plan of Correction: Principal immediately locked the cabinet during this inspection and will retrain staff on keeping personal items locked at all time.
Standard #: 22VAC40-185-340-G Description: Based on interviews with staff and incident report, a child under 10 was found not within sight and sound supervision guidelines. Evidence: On 7/18/2016 a 4 year child was left in the intermediate classroom alone for approximately 10 minutes not within sight and sound supervision. Staff #1, led the group of children outside to the playground prior to doing face to name, Staff #2 was suppose to bring Child #1 outside however she forgot to get the child and proceeded to the playground. Plan of Correction: Principal retrained the staff on face to name training and will continue to monitor the teachers on the protocol for maintaining supervision.
Standard #: 22VAC40-185-550-E Description: Based on review of the monthly fire drill log, the center did not have documentation of a monthly drill for the following month. Evidence: No documentation of a fire drill for the month of June 2016. Plan of Correction: Principal will make sure that a fire drill is completed and documented each month.
Standard #: 22VAC40-185-560-G Description: Based on observation of the food brought from home, several did not contain the child's name and date. Evidence: 1 lunch containers did not include the child's name and or date. Plan of Correction: Principal stated she will retrain staff.
