Comments:
An announced complaint inspection was conducted on May 19, 2016 regarding an allegation involving supervision and forbidden action. The investigation was conducted jointly with Child Protective Service and the Police Department. Licensing Inspector reviewed supervision, children's records, staff records, transportation policies and procedures and interviewed staff. On June 21, 2016 licensing inspector interviewed staff. Based on interviews, there is not a preponderance of evidence to support the allegation regarding supervision and forbidden action therefore the complaint is deemed NOT VALID. Additional violations were found that were not reported.

Description: Based on 1 of 5 records reviewed, the licensee did not ensure the records contained a physical examination. Evidence: During the complaint investigation conducted on May 18, 2016, the record for child #1 did not have a physical examination.

Plan of Correction: Child no longer attends facility.

Complaint related: No
Description: Based on 2 of 5 records reviewed, the licensee did not ensure the each child's record contained documentation of required information. Evidence: During the complaint investigation conducted on May 18, 2016, in the record for child #2 and child #3, did not contain a documented start date.

Plan of Correction: New registration was done start date wasn't transferred over.

Description: Based on the record review and interview with staff, the licensee did not ensure before the child's first day of attending , parent was provided in writing the transportation policy. Evidence: During the complaint investigation on May 18, 2016, Staff#2 stated that parents give verbal permission for transportation. The center does not give parents written policies for transportation policy. Staff #2 stated that some of the policies are reviewed during the meeting with the parents. Staff #2 was unable to provide the licensing inspector with the written transportation policy.

Plan of Correction: Transportation policy was reviewed with inspector ad a new policy is beginning prepare should be completed by 8/1/2016

Complaint related: No
Description: Based on observation and interview, the center did not ensure that emergency number is in the vehicles that transport children. A 911 or local dial number for police, fire and emergency medical services and the number of the regional poison control center is posted in a visible place. Evidence: 1. During the complaint investigation on May 18, 2016, staff #2 stated that she did not have emergency numbers in the center vehicles when transporting children.

Description: Based on van inspection and interview, the center did not ensure a prepared document containing potential shelters and evacuation routes, etc., that pertain to each site frequently visited or of routes frequently driven by center staff for center business (such as field trips, pickup/drop off of children to or from schools, etc.). This document must be kept in vehicles that centers use to transport children to and from the center. Evidence: 1. During the complaint investigation conducted on May 18 , 2016, staff #2 stated there were no documents in the vehicles containing written information regarding potential shelters and evacuation routes that pertained to each site frequently visited by the center for pickup/drop off of children to and from schools and home.

Description: Based on record review and interview, the licensee did not ensure parental permission for transportation. Evidence: During the investigation on May 18, 2016, staff #2 stated the center did not have documentation of parental permission for transportation.

Description: Based on 3 of 4 records reviewed, the licensee did not ensure staff records have documentation of updated background checks completed within the past 3 years. Evidence: During the inspection on May 18, 2016, the record for staff #1's most recent criminal history check completed June 17, 2011. Staff #3 most recent most recent criminal history check completed March 21, 2013. Staff #4 most recent criminal history check completed April 4, 2013.

Plan of Correction: Except for one that need to be notirized. All criminal history checks were done and checked by inspector on 7/06/2016.
