Technical Assistance:
Discussed: Not using the Vshaped Infant equipment that was brought in by a parent.

Comments:
Unannounced Renewal Inspection was conducted beginning at 9:30 and ending at 12:15. There were 50 children in care at the center and on a Field Trip to the pool today. There were 35 children at the center and 15 on the Field Trip. There were 8 staff . Ratios were appropriate in each age group. Children were observed playing outside, at circle time and listening to a story. Babies were observed being fed and played with. Emergency escape drills are up to date as is the Health Inspection. Fire Inspection needs to be updated. There are no children on medication at this time. Reviewed 8 children's files - complete Reviewed 4 staff files - see violation notice for Independent Contractor file violation. Violations:
Standard #: 22VAC40-185-260-A
Description: Based on a review of Inspections, it was determined that there had not been a Fire Inspection in over one year. Evidence: The Date of the last Fire Inspection was 7/21/15.

Plan of Correction: The Assistant Director will call the Fire Marshal to request a new inspection.

Standard #: 22VAC40-185-330-B
Description: Based on observation, it was determined that there was not sufficient resilient surfacing in the fall zones of the outdoor play structures. Evidence: There was less than 2 inches of resilient surfacing in the fall zones of of the two play structures. Mulch is washing away under the gates and some areas of the fencing.

Plan of Correction: Management will have someone look at the playground to determine a permanent fix to the fall zones and the resilient surface washing away in storms. In the mean time, mulch will be raked and maintained in the fallzones.

Standard #: 22VAC40-185-330-F
Description: Based on observation, there is no shade on either playground. Evidence: At the time of observation, 11:30 a.m., there was full sun on both the large playground and the toddler/twos playground.

Plan of Correction: A shade structure will be used on both playgrounds.

Standard #: 22VAC40-191-50-A-1-a
Description: Based on observation in classrooms and interview with staff, it was determined that there was a person in a classroom alone with a child that didnot had their Criminal Record Check on file. Evidence: Staff A, who is working with a Special Needs child at the center, and who is paid by the County, was alone with one child in the classroom. His Background checks had been sent for but had not yet been received. Date of Hire was 7/19/16.

Plan of Correction: The Assistant Director will call the Virginia State Police and check on the Form. Staff A will not be left alone with any children until the criminal Record Check is received.
