An unannound renewal inspection was conduced today 8/23/2016. The inspector arrived at 8:10 A.M. and departed the program at 4:20 P.M. There were one hundred forty-six children present with twenty-five direct care staff members. During the inspection all eleven classrooms were observed, lunch time was observed, infant care was observed, diapering was observed, hand washing was observed, nap time was observed emergency supplies were viewed, all seven medications along with authorizations were reviewed, afternoon snack time was observed, policies and procedures were reviewed, and a sample of injury reports were reviewed.
Standard #: 22VAC40-185-280-B Description: Based on observation, the center failed to ensure that all chemicals were locked. Evidence: 1. The inspector found in the purple ponies I room several spray cans of RID lice spray was in the closet that is required to be locked. 2. In the Red Bear I room there was a container of air freshener that was unlocked in the closet. 3. In the outside building a large spray container was unlocked. Plan of Correction: All staff will be reminded to properly store all hazardous material in a locked cabinet. The lice spray and air fresheners will be moved to a locked storage areas by the end of the day and the outside building was locked at the time of the inspection.
Standard #: 22VAC40-185-310-B Description: Based on observation upon arrival into the center, the center failed to ensure that each classroom had 25 square feet per child. Evidence: 1. The inspector found 24 children in in the Blue Bunnies I classroom. The classroom measured 22.3x18.11 for a total of 403.85 square feet. This space can have 16 children. Plan of Correction: The class size will only be 16 children in order to meet the square footage standard. Staff will be reminded to take their class to their own rooms as they arrive for their shift in order to maintain compliance.
Standard #: 22VAC40-185-340-F Description: Based on observation of the infant room, the center failed to sure children under 10 years of age always shall be within actual sight and sound supervision of staff. Evidence: 1. The inspector found at approximately 10:45A.M. in the Little Lambs II room two infant sleeping in the crib room. One staff was sitting in the rocking chair not able to see the sleeping children. The other staff person was walking around the sink area taking care of the other infants. There were a total of seven infants in care in this room. 2. Approximately at 11:30A.M. the Licensing Administrator and the inspector observed in the Little Lambs II room one infant sleeping in the the crib room. One staff person was sitting in the rocking chair and the second staff person was standing behind the changing table. Both staff were not able to see the sleeping infant. Plan of Correction: Staff working in the infant classroom will be reminded to make sure that all children are within sigh/sound supervision at all times. Staff will be instructed to always position themselves and/or the cribs in a manner so that they can maintain compliance.
Standard #: 22VAC40-185-440-B Description: Based on observation of the infant room, the center failed to ensure that all cribs, cots, rest mats and beds were identified for use by specific child. Evidence: 1. The inspector found that in the Little Lambs III room that three cribs were not identified for use by the specific child. Plan of Correction: During the inspection the teacher labeled the cribs with tape.
Standard #: 22VAC40-185-500-A Description: Based on observation of the infant room, the center failed to ensure that all hands were washed according to the requirements. Evidence: 1. The inspector observed child# 1 eating morning snack and did not have his hands washed prior to eating or after eating. Plan of Correction: Staff will receive additional training on proper hand washing procedures with emphasis on the times when it is required to be done.
Standard #: 22VAC40-185-520-A Description: Based on review of the sunscreens, the center failed to ensure that all over-the-counter skin products shall be used in accordance with the manufacturer's recommendations. Nonprescription drugs and over-the counter skin products shall not be kept or used beyond the expiration date of the product. Evidence: 1. The inspector found in Purple Ponies II Children # 2 and 3 had sunscreen that had expired. Both expired on 7/16. 2. In the Pre-K room child # 4 sunscreen expired on 7/16. Plan of Correction: Staff will be reminded to review sunscreen expiration dates on a weekly basis. All sunscreens that has expired will be discarded by the end of the day.
Standard #: 22VAC40-185-520-B Description: Based on review of sunscreens, the center failed to ensure that all requirements for sunscreen are met. Evidence: 1. The inspector found in Red Bear I one sunscreen not labeled sitting on the shelf. 2. In the Blue Bunnies Room, there was one sunscreen that was not labeled. Plan of Correction: Sunscreen that is not labeled with a child's name will be labeled by the end of the day. Staff will be reminded to properly label all sunscreen when it's brought to the Center.
Standard #: 22VAC40-185-520-C Description: Based on review of diaper ointments, the center failed to ensure that all requirements for diaper ointments are met. Evidence: 1. The inspector found in the Little Lambs I room a Pasta De Lassra diaper ointment that expired 6/16 and was also not labeled with the owner's name. 2. In the same room for child # 5 diaper ointment expired 5/16. Plan of Correction: Staff will be reminded to review diaper cream expiration dates on a weekly basis. All diaper creams that has expired will be discarded by the end of the day.
Standard #: 22VAC40-185-560-F-4 Description: Based on observation of lunch time, the center failed to ensure that children three years of age or younger may not be offered foods that are considered to be potential choking hazards. Evidence: 1. The inspector observed lunch in the Red Bear I and II room (ages 2). Beans and hots dogs were being served. The hot dogs were cut into whole pieces and not minced as required for children three years of age of younger. Plan of Correction: The Center Cook will be reminded to prepare food for the younger children so that it does not present a choking hazard. Information regarding how food should be prepared/served will be posted in the kitchen at all times.
Standard #: 22VAC40-185-560-G Description: Based on observation, the center failed to ensure that all food brought from home was labeled and dated. Evidence: 1. The inspector found that a container of food in the Little Lambs III refrigerator that did not have a label on it or a date of when the food was brought in. 2. In the Little Lambs II room child # 24 food was not dated. Plan of Correction: Staff will be reminded to properly label and date all food brought from home.
Standard #: 22VAC40-185-570-E Description: Based on observation of the infant room, the center failed to ensure that prepared infant formula shall be refrigerated, dated and labeled with the child's name. Evidence: 1. The inspector found in Little Lambs III refrigerator three bottle for child # 23 that were not dated. Plan of Correction: Staff will be reminded to properly label and date all prepared bottles brought from home.
Standard #: 22VAC40-185-570-H Description: Based on observation, the center failed to ensure that prepared baby food not consumed during that feeding by an infant may be used by that same infant later in the same day, provided that the food is not served out of the baby jar and is dated and stored in the refrigerator; otherwise, it shall be discarded or returned to the parent at the end of the day. Formula or breast milk shall not remain unrefrigerated for more than two hours and may not be reheated. Evidence: 1. The inspector found a container of baby food for child # 25 with the date of 8/11/2016 on the day of the inspection of 8/23/2016. Plan of Correction: Staff will be reminded to either send home or properly dispose of opened baby food at the end of each day.
