An unannounced monitoring inspection was conducted on September 19, 2016. At the time of entrance there were 19 children and 2 staff members present with children. A sample size consisted of 3 children's records and the emergency information and health related information related to the job position for 3 staff members. Children were observed preparing for and eating snack and playing games. Snack consisted of broccoli, apples, milk and chicken nuggets,. The playground was not inspected due to inclement weather. The inspection began at approximately 3:40 p.m. and ended at 5:30 p.m.. Areas of non-compliance were noted on the violation notice. The results of the inspection were reviewed and verified with the center supervisor. Please complete the columns for ?description of action to be taken? and ?date to be corrected? for each violation cited on the violation notice, and then return it to the licensing office within 10 calendar days of receipt. Standard #: 22VAC40-185-350-E-5 Description: Based on observation and staff interviews, the licensee did not ensure that the ratio for children, from age of eligibility to attend public school through eight years was one staff member for every 18 children. Evidence: During the inspection on September 19, 2016, the child to staff ratio was inadequate for approximately 7 minutes. At 3:47 p.m. there were 50 school age children in care with only two staff members until 3:54 p.m. when another staff member arrived at the center. Plan of Correction: The center supervisor has adjusted the staff schedule to ensure all staff members are on site in cases of an early bus and to allow additional time for traffic.
Standard #: 22VAC40-185-560-F Description: Based on observation and staff interviews, the licensee did not allow all children to be allowed second helpings of food served. Evidence: During the inspection on September 19, 2016, 19 children asked for seconds of chicken nuggets and there was only enough available to serve 6 of the 19 children. Plan of Correction: The center supervisor will ensure there are second helpings available for participants that request them.
