An unannounced inspection was conducted on April 6, 2016 in response to a complaint received by the Department on March 30, 2016 regarding supervision, hand washing and program. Interviews were conducted with staff & administration, policies & procedures were reviewed, children's records were reviewed (see violation notice) and video surveillance was viewed for the days of 03/28/2016, 03/29/2016 and 04/01/2016. A classroom observation was also conducted. Based on the observations and information gathered during the investigation, the preponderance of evidence does not support the allegations. There fore the complaint is determined to be not valid. After the complaint investigations, the inspector conducted a monitoring inspection as a follow-up to the issuance of the provisional license. Children were observed napping and later having snack (see violation notice). Children had time to play with blocks and color a coloring sheet. Children was also observed playing on the indoor equipment. Per administration, the playground for the younger children is not in use until additional resilient surfacing can be installed. Three staff records were reviewed and classroom observation were made. Today's ratios were as follows: Two's room: 4 children with 1 staff Three's room: 12 children with 2 staff Four's room: 9 children with 1 staff The violation from the previous inspection on March 22, 2016 were checked for corrections. Standard #: 22VAC40-185-130-A Complaint related: No Description: Based on review of children's records, the facility did not obtain documentation that each child had received the immunizations required by the State Board of Health before the child attended the center. Evidence: 1. Child #1, with an enrollment date of 03/07/2016, did not have documentation of immunizations on record. 2. Child #2, with an enrollment date of 03/07/2016, did not have documentation of immunizations on record. Plan of Correction: Per the Director: We had requested that they provide of a copy of the immunization records but they did not. They are no longer enrolled.
Standard #: 22VAC40-185-60-A Complaint related: No Description: Based on review of children's records, the facility did not obtain all required information for each child's record. evidence: 1. The record of child #1 did not have a name, address and phone number listed for a second emergency contact. 2. The record of child #2 did not have a name, address and phone number listed for a second emergency contact. 3. The record of child #1 did not have documentation that proof of the child's identity and age had been viewed. 4. The record of child #2 did not have documentation that proof of the child's identity and age had been viewed. Plan of Correction: Per the Director; The children are no longer enrolled.
Standard #: 22VAC40-185-500-B Complaint related: No Description: Based on observations, the facility did not ensure that diapers were disposed of in a plastic lined storage system that is either foot operated or used in such a way that neither the staff member's hand nor the solid diaper touches the exterior surface of the storage system. Evidence; Staff #1 was observed lifting the diaper pail lid with her hand to throw away a diaper after diaper a child in the two year old classroom. Addition staff were observed lifting the diaper pail to throw away trash from snack time. Plan of Correction: Per the Director: I will review our procedures with staff again to ensure they use the foot pedal and not their hands to open the diaper pail.
Standard #: 22VAC40-185-560-G Complaint related: No Description: Based on observations, the facility did not ensure that food containers brought from home were sealed and clearly dated and labeled in a way that identifies the owner. Evidence: Five of the lunch boxes in the two year old classroom were not labeled with a date and name to identify the owner. Plan of Correction: Per the Director: we will make labels for each lunch box with the owners name and then date daily.
Standard #: 22VAC40-185-560-K Complaint related: No Description: Based on observations and interviews, the facility did not sanitize the tables before and after each use for feeding. Evidence: The inspector observed the children in two year old classroom sitting down and starting to eat their snack however the tables had not been sanitized before hand. Staff stated they had cleaned them earlier in morning after taking the chairs down. Plan of Correction: Per the Director: I will review with staff the procedures to ensure tables are sanitized before and after each meal.
