Comments:
A renewal study was conducted from 9:30 am - 11:30 am. There were 25 children in care being supervised by 6 staff members. The children were observed participating in story time and table top activities. Lunch served today is pancake, yogurt, turkey sausage, orange slices, and milk. Seven children's records were reviewed. Four staff records were reviewed and in compliance. The most recent emergency escape drill was conducted 9/22/2016 and the most recent shelter in place drill was conducted 2/24/2016. Seven medications were reviewed. There are currently four staff members on site who are MAT (Medication Administration Training). Standard #: 22VAC40-185-130-A
Description: Based on observation of seven children's records it was determined that not all records contained documentation of immunizations before the child's first day of enrollment. Evidence: Child D with a start date of 8/29/2016 was missing documentation of immunizations.

Plan of Correction: Administration will request the most recent immunization record from the parents of child D.

Standard #: 22VAC40-185-140-A
Description: Based on observation of seven children's records it was determined that not all records contained documentation of a physical with the first thirty days of enrollment. Evidence: Child D with a start date of 8/29/2016 did not have documentation of a physical on file.

Plan of Correction: Administration will request the most recent physical from the parents of child D.

Standard #: 22VAC40-185-60-A
Description: Based on review of 7 children's records it was determined that not all records were complete. Evidence: Child A and B with start dates of 7/18/2016 were missing the street address for the 2 required emergency contacts. Child D with a start date of 8/29/2016 was missing one of the two required emergency contacts. Child A and B with start dates of 7/18/2016, child C with a start date of 7/17/2016, and child D with a start date of 8/29/2016 were missing documentation of proof of identification.

Plan of Correction: Administration will request emergency contact information and proof of identifications from the parents of child A, B, C, and D.

Standard #: 22VAC40-185-510-G
Description: Based on observation of seven medications it was determined that not all medication on site was accompanied by a written authorization from a parent. Evidence: Child E (Age 10) had a medication on site that did not accompany a parental authorization form.

Plan of Correction: Administration will request an authorization form to be completed for the parent of child E.
