An unannounced renewal study was conducted from 10:15 a.m.-12:15 p.m. with the center owner. 34 children, ranging in age from 2-5 years old, were under the care and supervision of five staff. Children were observed building with Duplos, coloring, playing with trains, and eating lunch. Lunch served today was hamburger pizza, mixed vegetables, applesauce, and milk. Required information was posted. The health inspection, fire inspection, and liability insurance are all current. The complete first aid kit, battery operated radios, and battery operated flashlights were present and accessible. The last emergency escape drill was 9/20/16. The last shelter-in-place drill was 2/18/16. The outdoor play area was inspected. Two medications were reviewed and in compliance. Two staff 's and four children's records were reviewed. Standard #: 22VAC40-185-150-B Description: Based on review of four children's records, the center did not have immunization and physical examination documentation that was signed or stamped by a doctor, his designee, or local health department official. Evidence: Child A's immunization documentation and physical examination were printed pages from Kaiser Permanente and were not signed or stamped. Plan of Correction: We will be very aware and mindful of physician signatures on immunization and physicals.
Standard #: 22VAC40-185-160-A Description: Based on review of two staff records, the center did not have documentation of a tuberculosis (tb) test or screening for staff within 21 days of hire or within 12 months prior to hire. Evidence: Staff A's (hired 8/17/15) tb test was dated 2/8/16. Plan of Correction: I believe that Staff A did have a tb test immediately after she started. Documentation of that tb test may have been misplaced in the office.
