Standard #: 22VAC40-185-150-B
Description: Based on review of four children's records, the center did not have immunization and physical examination documentation that was signed or stamped by a doctor, his designee, or local health department official. Evidence: Child A's immunization documentation and physical examination were printed pages from Kaiser Permanente and were not signed or stamped.

Plan of Correction: We will be very aware and mindful of physician signatures on immunization and physicals.

Standard #: 22VAC40-185-160-A
Description: Based on review of two staff records, the center did not have documentation of a tuberculosis (tb) test or screening for staff within 21 days of hire or within 12 months prior to hire. Evidence: Staff A's (hired 8/17/15) tb test was dated 2/8/16.

Plan of Correction: I believe that Staff A did have a tb test immediately after she started. Documentation of that tb test may have been misplaced in the office.
