Discussion was held on the topics of current enrollment and current staffing.
The following inspection took place between the hours of 11 am through 2 pm. Today, I met with the Staff of the Kiddie Academy of Centreville located in Centreville, Virginia. At the start of this unannounced monitoring inspection there were a total of 92 children with 18 classroom staff; good ratios in all 6 rooms. A total of 5 children?s files and 7 new staff files were reviewed. The children were engaged in the following: diaper changing, tummy time, hand washing, infant feeding, learning patterns, numbers, writing one's name, large motor skills on the playground, games, art, lunch, nap time, and other organized activities. Standard #: 22VAC40-185-130-B Description: Based on review, 1 of 5 children's files reviewed indicated that the center did not obtain documentation of additional immunizations once every six months for children under the age of two years. 1. On the date of inspection (10/11/2016), current immunization were not available for Child #1. 2. The most current immunization on file for Child #1 was dated - 03/10/2016. 3. On the date of inspection, Child #1 was listed to be under the age of 2 years. Plan of Correction: An updated immunization page for Child #1 shall be obtained and placed on file.
Standard #: 22VAC40-185-160-A Description: Based on review, 1 of 7 new staff files reviewed indicated that each staff member did not submit documentation of a negative tuberculosis (TB) screening no later than 21 days after employment. Evidence - 1. On the date of inspection (10/11/2016), TB documentation was not available for review for Staff #2. 2. The start date for Staff #2 was listed as - 08/11/2016. Plan of Correction: Initial TB documentation shall be obtained and placed on file.
Standard #: 22VAC40-185-60-A Description: Based on review, 1 of 5 children's files reviewed indicated that not all of the required documentation was contained within each child's record. Evidence - 1. On the date of inspection, the Proof of Identification for Child #3 was not available for review. 2. The start date for Child #3 was listed as - 06/06/2016. Plan of Correction: The Proof of Identification for Child #3 shall be obtained and placed on file.
Standard #: 22VAC40-185-70-A Description: Based on review, 4 of 7 new staff files reviewed indicated that not all of the required documentation was within each staff record. Evidence - 1. On the date of inspection (10/11/2016), education documentation was not available for: Staff #1, and Staff #2. 2. At least two complete references were not available for review for the following: Staff #2, Staff #3, and Staff #6. Plan of Correction: All missing documentation shall be obtained and placed in the applicable staff file.
Standard #: 22VAC40-185-440-J Description: Based on observation, there was not at least twelve inches of space between the ends of each occupied crib. Evidence - 1. On the date of inspection (10/11/2016), within the infant room at approximately 11 am., three cribs were observed end to end to end with less than 12 inches of space between each one. 2. All three cribs were occupied with a sleeping infant. Plan of Correction: Corrected on the date of inspection.
