Today an unannounced monitoring inspection was conducted beginning at 2:15pm. There were 40 children present today with 3 staff. Children were observed arriving from school, playing with various games or activities at the tables, washing hands, and eating snack. Four medications and authorizations for children enrolled in addition to 4 children's records were reviewed. Staff records are reviewed annually in the administrative office. Standard #: 22VAC40-185-40-D Description: Based on direct observation, the most current license is not posted in a place conspicuous to the public. Evidence: The license that is posted is an expired license which expired 10/15/15. The current license, dated with the effective dates of 10/16/15 through 10/15/17 is not posted. Plan of Correction: I will ask my supervisor to request a duplicate copy from the licensing office if we cannot locate the current license.
Standard #: 22VAC40-185-510-D Description: Based on a review of medications and authorizations, a medication authorization is not available to staff during the entire time it is effective. Evidence: One child (#1) with a prescription medication on-site does not have an authorization available for Loudoun County Parks, Recreation and Community Services. The authorization available is only for Loudoun County Public Schools to administer the medication. Plan of Correction: We will have the parents fill out the short-term authorization form today at pick-up. We will also request that they have a physician fill out a long-term medication authorization form specific to Loudoun County Parks, Recreation and Community Services.
