Comments:
An unannounced, mandated monitoring inspection was conducted today. The inspector arrived at 9:30 AM and departed at 12:15 PM. Breakfast service and morning activities were observed. A sample size of five children's records and four staff records were reviewed. Medication administration and injury records were reviewed. An exit meeting was conducted with the program director during which the facility's plan of correction was obtained for the violation cited. Please contact the inspector if there are any questions.

Violations:
Standard #: 22VAC40-185-510-A
Description: Based upon review of four children's medications and staff interview, the facility has not ensured that prescription medication is administered according to the facility's written medication policy and only with written authorization from the parent. Evidence: 1. Albuterol was administered to child 6 on 9/23/16. There was no written parental permission for this medication to be administered. 2. There was albuterol stored for potential administration to child 7 and to child 8. Written parental permission was not in the records provided for either child 7 or child 8. 3. Staff 6 was unable to provide documentation of written parental permission for medication to be administered to child 6, 7 or 8. 4. The facility's policy for medication administration indicates that written parental permission is obtained prior to accepting or administering medication.

Plan of Correction: The facility responded that written parental permission will be obtained prior to any medication being stored or administered to children in care.
