Comments:
An unannounced monitoring inspection was conducted on Thursday, October 20, 2016, from approximately 9:45am until 11:15am. There is one classroom that had fourteen children in attendance with three staff. The children were working with trinomial cubes, completing puzzles, spelling words, completing math sheets, discussing how many syllables were in a word, self-serving snack and working with number counters. Outside the children have a garden growing lemon balm, cabbage, rosemary and mint.
A practice evacuation drill was conducted 10/5/16.
Snack: Bananas and crackers
All required items to be posted were present. Emergency supplies were checked and complete.
Three staff records and three children's records were inspected.

Violations:
Standard #: 22VAC40-185-270-A
Description: Based on inspection of the facility, the center did not ensure all areas and equipment , inside and outside, were maintained in a safe and operable condition. Evidence: 1. There is a wooden pyramid structure that has deteriorated and has sharp edges. 2. The landscaping paper has lifted up and is posing a tripping hazard. 3. A wooden landscaping timber poses a tripping hazard where it is located by the sandbox.

Plan of Correction: Per Center: "Center will remove tripping hazards as soon as possible. Will remove pyramid structure and not allow students to play on until removal."
