Comments:
An unannounced monitoring inspection was conducted between 8:30am and 9:15am. There were 39 children in the direct care of 2 staff members at the beginning of the inspection. Upon the exit interview there were 45 children in the direct care of 3 staff members. Children were observed preparing for a flag football tournament, reading, playing with electronics, and doing an art project. The center is decorated with a 70's theme. A sampling of children's records, the evacuation drills, medications, authorizations, physical space, and supervision were observed and inspected. Violations:
Standard #: 22VAC40-185-350-E-5
Description: Based on observations, the center did not maintain staff ratios of one staff member for every 18 children. Evidence: At 8:40am there were 39 children in the direct care of 2 staff members. A 3rd staff member arrived at 8:46am to meet that state mandated ratio for children age 5-12 years old.

Plan of Correction: Will have staff come in earlier.
