Comments:
An unannounced monitoring inspection was conducted on November 14, 2016. At the time of entrance there were 2 children and 2 staff members present. The sample of records reviewed consisted of 3 children's records and the emergency contact information and health related information for 2 staff. Children were observed preparing for and eating snack and working on homework. hockey. The inspection began at approximately 2:55 p.m. and ended at approximately 4:30 p.m.. Areas of non-compliance are identified on the violation notice. The inspection was amended on 11-15-16. If you have any questions, contact your licensing inspector at (757) 404-0568.

Violations:
Standard #: 22VAC40-185-260-A
Description: Based on documentation review and staff interviews, the licensee did not ensure there was an annual fire inspection report from the appropriate fire official having jurisdiction. Evidence: During the inspection on November 14, 2016, the most recent inspection was dated 6-25-2015. The inspection has expired as it is required annually.

Plan of Correction: The center director will have the documentation of inspection within the week.

Standard #: 22VAC40-185-260-B
Description: Based on documentation review and staff interviews, the licensee did not ensure there was an annual approval from the health department. Evidence: During the inspection on November 14, 2016, the most recent inspection was dated 7-7-2015. The inspection has expired as it is required annually.

Plan of Correction: The center director will work the supervisor to contact the health department for the inspection.

Standard #: 22VAC40-185-500-A
Description: Based on observation and staff interviews, the licensee did not ensure children's hands were washed with soap and running water or disposable wipes before and after eating meals or snacks. Evidence: During the inspection on November 14, 2016, two children were observed eating snack and neither child washed thier hands after eating snack.

Plan of Correction: The center director will have children wash their hands after eating.

Standard #: 22VAC40-185-560-F
Description: Based on observations and staff interviews, the licensee did not ensure that when the center provides food that the most recent, age-appropriate nutritional requirements of a recognized authority such as the Child and Adult Care Food Program of the United States Department of Agriculture (USDA) were followed. Evidence: During the inspection on November 14, 2016, the requirements for serving food were not met as they relate to USDA. The serving size required to meet the USDA standards for serving milk were not met for the age group 6-12 as they are required to have 1 cup of fluid milk and they were served and only 1/2 cup of fluid milk.

Plan of Correction: The center director will purchase larger cups to serve more of the beverage on the menu.
