Comments:
The licensing inspectors conducted an unannounced renewal inspection on November 16, 2016 from 1:45 pm to 4:00 pm. The Director was present for the inspection. Upon the inspector's arrival, the staff was observed preparing snacks for their school-aged children. During this inspection, there were observations completed of the various groups of children throughout the center. There was a total of 36 children and four staff present for this inspection. The first through third grade group consisted of 15 children and 1 staff. The children were doing homework, and reading. The fourth grade group consisted of 12 children and 1 staff and they were in the art center. The fifth and sixth grade group included 9 children and 2 staff. The children were engaged in art. The licensing inspectors also completed playground inspection. The posting requirements were observed and in compliance at the center. There were 3 children?s records and 3 employee's records reviewed during this inspection. Medication is administered and was reviewed. The center's injury reports were reviewed as well as the center?s injury prevention plan which was updated. The center's first aid kit and emergency supplies were inspected. An application for renewal of the license has been received by the Department. Description: Based on a review of 3 children's records, the center did not obtain a physical examination for a child within one month after attendance. Evidence: The record for Child#3 did not include a completed physical examination form.

Plan of Correction: I will get with child's parent and ask her to give updated physical exam form.
